Starting in 2015, the state required a change in the acknowledgment and jurat. The notary disclosure has very specific
wording and is required to be surrounded by a box. Sometimes this wording is called the notary box disclosure. The
idea behind the wording is to make it clear that the notary is not approving the legality of the document when it is
notarized but is only identifying the signer. Prior to this disclosure, some signers thought that the document became
an enforceable legal document simply because it was notarized.
Most current forms in California have the correct wording on the form. However, there are still forms where the
wording is missing or incorrect. The acknowledgement disclosure stamp allows you to stamp the wording and the
surrounding box on the form and comply with California requirements.
